The lower rotating arm began rotating and crushed the employee between the print head and the arm. Employee#1 sustained internal injuries to his abdomen and was hospitalized.

NarrativeAt 3:00 p.m. on October 17, 2017, an employee was cutting material using a DHS 790 sliding miter saw. The saw faulted while cutting and pulled the piece of material into the blade. The employee's hand was holding the material close to the point of operation. The employee's thumb sustained a laceration to the bone.
