NarrativeAt 1:00 p.m. on October 16, 2018, an employee was speaking to another employee working on a door on the "surgery" table in the customs department. At the same time, another employee was operating a table saw approximately 10 feet away, with the operator/back end of the saw facing the employee. The operator was performing rabbeting on a piece of molding when a the circular saw blade caught the end of the piece of wood, catapulting it behind the operator into the employee's chest. The employee sustained a penetrating injury to the chest including injuries to the heart and right lung leading to exsanguination. The employee was hospitalized and later died.
OSHA investigators arrived in Colton, CA on Wednesday, October 31st 2018 and began an accident investigation after learning of an incident at C & K Form Fabrication Inc which had occured on Saturday, July 7th 2018. A 54 year old Laborer had been killed when the employee died from heat illness

NarrativeAt 3:00 p.m. on July 7, 2018, Employee #1 completed his work shift and left the worksite in his personal vehicle. Employee #1 was responsible for the overall housekeeping of the lumber yard and to assist other employees with the staging of materials and finished product. He was discovered unresponsive in his personal vehicle offsite hours later by a passerby. The cause of death was listed as: Hyperthermia due to high environmental temperature hours. Employee #1 was working in direct sunlight under heat wave conditions for two consecutive days.

NarrativeAt 8:32 a.m. on January 22, 2018, an employee was standing near the end of a conveyor belt and had rested his right hand near a moving chain belt while wearing a glove. The employee's glove was pulled into the chain conveyor and crushed both his index and middle finger in a nip point, later amputating each of them from his hand. The employee was hospitalized.
