NarrativeAt 10:45 p.m. on December 3, 2017, an employee was retrieving an air lance from a moving Larry Car and was caught between it and a stationary object. The employee suffered a fractured spine and was killed.

NarrativeAt approximately 7:45 p.m. on August 8, 2017, an employee was unjamming material as it was passing from one section of the machine to the cutting section. The employee opened an interlocked door, placed a 4.5-inch by 1-inch size magnet on the interior part of the interlock system, placed his left hand near the scissor blades when the machine, in jog mode, started up unexpectedly, cutting off the tip of his left thumb. The employee was hospitalized and treated for his injuries.

NarrativeAt 4:30 p.m. on June 4, 2017, two employees were removing a jam in a coal bin. Employee #1 and Employee #2 were crushed when the weld on the coal bin cone broke away and buried him underneath the coal. No specific injuries or deaths were identified in the initial report. However, one employee was hospitalized.
