NarrativeAt 6:00 a.m. on April 4, 2019, an employee died from abdominal injuries. The employee initially sustained his injuries on March 21, 2019, when he stepped on a drag chain conveyor pan to dislodge a stick and became caught. The employee was pulled into the equipment's tall spool by his leg. His leg was amputated and he also sustained abdominal injuries.

NarrativeAt 10:15 p.m. on January 27, 2019, an employee was cleaning molds with a hand drill and a wire brush while inside a TPM Line 1 Machine. The employee was caught in the machine when it activated and the molds closed around him, crushing the employee and fractured his skull and face. The employee was transported to the San Joaquin General Hospital where he was pronounced dead.

NarrativeAt 11:00 a.m. on December 29, 2017, an employee was observing five baggers when the film for bagging broke. The employee reached into the machine while troubleshooting and one of his fingers contacted the machine's hot knives. His fingertip was amputated as a result.
